Choices For Independence


The Choices for Independence program is a program by the New Hampshire Department of Health and Human Services, specifically designed to cater to the needs of adults aged 18 and older who require nursing home-level care. However, it allows individuals to maintain their preference of living at home.

Services We Offer Through This Program

Skilled nursing-(Check Vitals, Fill Med Box, assess and monitor)
LNA(Bathing assistance, Dressing)
PCSP( Personal Care Service Provider) (Medication Reminders, Laundry, Meals, Errands)
Homemaker (Light housekeeping)

How to Apply

Option 1: Online Application
Go to 
www.NHEasy.NH.gov and complete the application online.
Option 2: ServiceLink
Contact your local ServiceLink Office at 
1-866-634-9412 or online at www.servicelink.nh.gov. A ServiceLink Options Counselor will work with you in person or over the phone to help you apply. If you do not qualify for CFI the ServiceLink Options Counselor will discuss other options with you.
Option 3: Your Local DHHS Office
Go to your local DHHS Office. To find the office closest to you call 
603-271-9700 or go to their website at www.dhhs.nh.gov.

Medically eligible for nursing home placement

Meet Medicaid income and asset guidelines
